Claude Code says it failed to connect
Type /mcp in Claude Code, or run claude mcp list in a terminal, to see why. The message after Failed to connect tells you which case you have.
HTTP 401, with "A valid VSTA API token is needed." VSTA did not accept the token in the connection. That happens when:
- the connection was removed on the Connect Claude page;
- you were removed from the client, or are no longer a facilitator of it;
- the command was copied only in part.
Make a new connection. Then run claude mcp remove with the old connection's name and -s user, such as claude mcp remove vsta-ledgerline -s user, and run the new command. If you are no longer a facilitator, ask a facilitator of the client.
ECONNREFUSED, or "Unable to connect". Claude Code cannot reach https://vsta.work. Check that you are online. Then check that the command you ran had https://vsta.work/mcp in it, and make a new connection if it did not.
HTTP 403, with "This origin may not call VSTA." A web page on another site tried to call VSTA. Claude Code does not cause this, so you should not see it. If you do, tell the person who looks after VSTA.
HTTP 413, with "The request is over 4 MB." Claude sent more than VSTA takes in one request. An import of a few hundred practices fits well inside the limit, so this usually means the document holds far more text than it should. Ask Claude to check what it put in the document.
Claude says nothing with that id is in the map
Claude used an id VSTA does not know in this client's map. The record may have been deleted, or the id may belong to another client. Ask Claude to look the record up again with search or get_organisation and use the id it finds.
A change in a list was refused
run_commands makes changes in order and stops at the first one VSTA refuses. Changes before it are saved. The refused change and everything after it are not made. Claude gets the reason, such as a stage that still holds practices. Ask Claude what was refused and why, fix that, and send the rest again.
The import will not apply
apply_import writes nothing while the report lists problems. Ask Claude to fix the problems and preview again. Read the import preview explains each part of the report.
VSTA couldn't do that just now
Claude says VSTA couldn't do that just now when VSTA could not reach the client's map for a moment. Ask Claude to check what was done, then try the rest again.
